Hmm...what is it exactly about the right shoulder (our right) that looks off? There's foreshortening going on; in addition to being raised higher, that arm is comming slightly forward.
The cloth behind the hand with the heart is called a cravat. It's that poofy, flowing garment that men wore around their necks in the Victorian era, usually tucked underneath their jackets. AKA, the predescessor to the modern-day tie.
