Heart Bound - A Medical Drama
'All Staff report to the Emergency Room immediately' sounded the receiver I got up and ran out the door, down the hall, down two flights of stairs and to the ER room as fast as I could, I got there and thrust the door open to reveal all of the members of staff to already be there.
"Kana! You are late!" bellowed the head surgeon of Cardiology, Akira-sensei, "I'm terribly sorry sir". I apologized sincerely, yet he just looks at me, tuts and begins the talk again. "An emergency operation has been arranged, the information is:
Name: Fujisa Kaito Gender: Male Age: 57 Illness: Heart Attack Operation: Coronary Artery Bypass
Right onto the Surgeons, we will have Hideki-sensei run this operation and he will need a good team, let's see Haruka-sensei you will be the Instrument's specialist, Daisuke-sensei please assist Anesthetics Zakura-sensei you will be the head assistant and finally we will have Mari-sensei as a backup assistant, that is all please prep and be ready in 15 minutes, got it! GO!".

The room cleared 'ahh I was left out again' I sighed and got up, I strolled to the door, as i opened it Akira-sensei was standing outside, I walked out and shut the door behind me, I then turned around to face him and pouted at him annoyed, "why are you pouting at me like that?" he asks with a 'I already know but I'll ask anyway look on his face', "No reason!" I reply sharply to him, before walking down the corridor away from him, "Ohh so that's how it is, is it, so you don't want to watch the operation from the observation deck?" he shouted after me, I turned around and flashed him a smile "no thanks!! I have important work!!" I shout at him.
I lied. An operation is fascinating to watch, I did want to watch, but... I was also tired of it just sat watching all of the time. I always ran after Akira-sensei's back, dreaming and hoping to be a genius surgeon like him, to become the best is all I wanted yet... something stopped me. Akira-sensei the one I looked up to, said I lacked something to do operations... but what was the thing I was lacking so much?, this question remains with me even now 2 years after entering this hospital. When I was first admitted here I admit I was just a young girl with a big dream, I was assigned to the Cardiology department, and had to serve under Akira-sensei, yet he said something to me which gave me the resolve to be like him, he said that I had alot of potential to be an amazing surgeon someday, I was so glad to hear words like those coming from someone as great as him, yet as a rookie about half a year in I made a mistake which has come to haunt me even now. I will overcome this one day.

I reach to open the door, and I hear laughter, the kids ward was always full to the brim of laughter and yet, the kids in here... Their bodies were forever braking down... they were dying slowly but not painfully, all beside two young boys named Yusuki and Koichi. The children who were dying laughed a lot more than Yusuki and Koichi though, they were truly happy given their circumstances, they were not happy because they were going to die though, it was more like they were happy for the time they had left to live. Those kids were the ones who had no chance to go to school, no chance to live with their families, no chance to run about free, but they had friends, because they had each other. Even though they were like this, being nothing but birds in a cage. They were truly happy.
I burst into the room clipboard at the ready, "okay who am i going to eat today" I growl like an animal at the children, the children just laugh in hysterics at me. "Today it's Koichi's turn" said Yusuki pointing at Koichi in the corner of the room sleeping, I walked over and poked him "Ko-kun check-up time" I walked back to the door as he got out of bed still tired I pointed at him and wiggled my finger "follow me" I said in a low voice once again, making my finger turn to point out of the door. Koichi walked over and followed me out of the room.
I scanned the corridor and tried to look for a check-up room but they all were full, yet there was a nurse's room that wasn't being used. So we borrowed it. I unlocked the door and invited Koichi in, "okay take a seat here" I offer him the seat opposite me, he sits down reluctantly "right check-up time" I say placing the stethoscope to his tiny bare chest, I listened to his heart for a minute, yet his pulse had become irregular from yesterday, 'why is this' I though shaking my head... 'NO!... it can't be...'. But it was Koichi had taken a turn for the worst.

I banged loudly on the observation deck's door and just as I thought, Akira-sensei opened it and looked at me "did you really want to come watch that much" he snickered pulling me into the room, for a moment I forgot why I came here, and looked down to the operation below 'why has it not started!' I looked at Akira-sensei with a puzzled look, and as I expected he answered "we were waiting on you", 'me, why' I looked at him in disbelief, this was an emergency operation, not a game, the patient's life was in danger, yet they could happily just wait on me!!, "Why!! this patient's life is in danger!!?" I questioned him furiously, "because.." he trailed off "you need to see this one". 'Is he for real!! this one?, how is it any different than the last 60 plus I had watched how really' I though almost wanting to punch him, "there is no other reason and i would not do this for anyone else, you NEED to see this surgery" he begged. Suddenly i remember why I came Koichi-kun.
"Unfortunately I do not have time for this" i scold him, "why?" he questions me, "because Koichi-kun has taken a turn for the worse" I say defiantly staring into his eyes, "how?" he questions the maturity of an adult returning to his voice 'finally' I thought 'his playful questioning has stopped, he's starting to take me seriously' he was still staring at me, and if I didn't know him any better I would say he's waiting for a diagnosis, "his heartbeat has become irregular from yesterday's, it is most likely bradycardia" I say then continue finish my prognosis with "he may have come in with not just a lung defect but also the sick sinus syndrome", I keep calm as not to ruin my composure at this minute, even though i would much rather cry for the unfortunate little boy.

Akira-sensei looked at me, "okay he will need an emergency operation right now! we will need to implant a permanent pacemaker to help his heart out, I need a good team let me see, Kana-chan I need you to hurry and bring me the following: I need Sakurai-sensei as my Anesthetic manager, Mai-sensei as my instrument specialist Heneri-sensei as my head assistant and Nana-sensei as my backup assistant, go hurry and gather them, then come back and watch this operation, please".
I ran to the nursing department first, to find Mai-sensei lounging about on her chair bored, I smiled a slightly evil smile, "Emergency operation!! You have been summoned by Akira-sensei!!" I shouted into the small room, it sort of echoed, my bad. Mai-sensei fell off her chair in a daze, and, got up just as quickly, "got it" she said the shock still present on her face as she came out of the room and rushed over towards the ER, I snickered and headed off again to find my next person.

My next stop was to the anesthetics department, when I got there and opened the door, there were several surgeon's discussing operations with the anesthetic doctors there. I scanned the room 'found him', smiling I ran to the corner of the room to see Sakurai-sensei eating his dinner, "Sakurai-sensei, there is an emergancy operation and Akira-sensei asked me to summon you" I reported to him, looking round the other two anesthetic doctors looked jealous "ahh I best be on my way then Kana-chan" replied Sakurai-sensei petting my head fondly, "also you can have this" he said handing me his instant noodles and chopsticks, i give him a grateful smile and walk towards the door with him, we left the room and I closed the door. "They are jealous of you?" I questioned him, he shrugged "well since there are only 4 anesthetic doctors in the whole hospital we are quite rare, and technically, me and Daisuke-sensei are the top graduates and are acknowledged by Akira-sensei, so that makes the lower two a little upset I guess" he smiled at me as he finished "you too Kana-chan", with that said he walked away to the ER. 'Me too, what?' does he mean Akira-sensei acknowledges me too... I doubt that.
I ran toward the head surgeon office and hoped I would be able to find either Heneri-sensei or Nana-sensei, as I got there I heaved open the door, BINGO, they were both there! this was lucky for me, "You lovebirds will have to pack up because you have been summoned by Akira-sensei" I report, "ahh what for?" asked Heneri-sensei uninterested, "an emergency operation" I replied, "we will be off" said Heneri-sensei getting up suddenly, Nana-sensei also got up and walked toward the door. Heneri-sensei ran up behind Nana-sensei and grabbed her arm and ran toward the ER with her, I could hear him say to her "hurry up".

I returned back to the observation room and nodded to Hideki-sensei, with that the operation began, without a moment to lose. Daisuke-sensei started off by placing catheters attached with various lines into the patients arm, "fentanyl" he ordered, Haruka-sensei ran towards him with a bottle, "syringe" he said, a moment later a syringe had been placed in his hand, he carefully prepared the syringe with the correct amount of medication, he pushed the end of the needle into a rubber bung on the catheter "injecting" he reports slowly injecting the mixture into the lines. He threw the syringe in the bin, "bring me the propofol bottle and a new syringe" he said demandingly Haruka-sensei appeared with what he needed once again, he carefully measured our the propofol again, "injecting" he reports and he then slowly injects it, as it goes through the lines, the patient slowly lost consciousness Daisuke then gets a tube which goes through the patients nose, and another which goes down the trachea, these lines are known as endotrachial tubes, he then secures them, which in turn allows medical ventilation to begin. All throughout the surgery Daisuke-sensei's main priority he had to do was to inject a continuous yet slow flow of propofol, this would ensure that the patient would not awaken.
Zakura-sensei began to prep the body for surgery, firstly he performed a median sternotomy, and keeps the rib cage held open with retractors, this allowes the whole chest cavity to be seen, then he had to cut a graft out from the patient's leg, the vein graft they were cutting out would then be used to repair the damaged vein on the heart which in turn would allow the heart to beat regulary again as there would be no blockage. Zakura-sensei went to the patient's leg, "scalpel" he demanded, which seconds later was swiftly placed onto his outstretched palm by Haruka-sensei, he immediately made a 5' incision to the patients right leg, "metzen" he demanded once again, and without delay was passed them. He successfully 'harvested' a graft from the veins present in the leg. He then sew the leg up.

Daisuke-sensei nodded a signal toward Haruka-sensei who nodded back, before rushing towards the medical cupboards at the other side of the room. Haruka-sensei scanned the bottles labels carefully yet fast and pulled out the bottle she needed, and headed back to Daisuke-sensei grabbing a syringe on her way. Haruka-sensei passed the bottle and syringe to Daisuke-sensei "here" she said nodding, Daisuke-sensei measured out the correct dosage of Heparin through the syringe, "Injecting" he said, with that he syringed it into the lines that lead to the patient.
"Right prepare the CBP" ordered Hideki-sensei, 'huh? they are going to do an on-pump surgery?', for a moment I wondered to myself 'why they would go to all this trouble to perform an on-pump surgery when they always do off-pump surgery's ', then it hit me they were doing it for me. I had been entered into the doctor exam earlier on in the week, in the doctor exam you must perform bandaging, CPR training, and two operations. The two operations we were told to do this year were both cardiopulmonary bypasses, one off-pump and the other on-pump.

"Cannule" Hideki-sensei orders, Haruka-sensei grabs it from the box on the side and places it in his hand, carefully he stitches it into the patients heart. "Attach the CPB Nana-sensei" he instructed to her "right away" she said pulling the machine to the side of the operating table, she attached the CPB machine, "now wait" Hideki-sensei instructed. "Zakura-sensei make sure that the aorta is now clamped shut" said Hideki-sensei, "certainly" Zakura-sensei replied, he held out his hand to Haruka-sensei "aortic cross-clamp" he ordered, Haruka-sensei sifted precisely through the instruments present and handed the correct one to Zakura-sensei. Zakura-sensei then clipped the aorta "done" he reported to Hideki-sensei with a nod.
As I sat there watching I finally came to realize something... why Akira-sensei wouldn't let me operate, what I lacked that caused my mistake a year ago. The speed of the commands, the speed of the instrument passing, the speed of... their teamwork. The reason I made the mistake is because I didn't think once about teamwork, throughout the operation I was focused only on myself doing the job correctly, that was the major turn in my mistake. I never noticed team work at all before but that was because an off-pump operation was usually much faster than an on-pump one, watching them for this amount of time has made me aware of the team work that they have, Akira-sensei wanted me to see this because of that fact, more so than because of the doctor's exam. I swore upon seeing this operation, that I would change, to be the best doctor.

"Nana-sensei start up the CPB" Hideki-sensei said, "starting the CPB now" she reported "cooling the blood to 29 degrees Celsius". "Heart stopped" Daisuke-sensei reports.
"Now it's a race against time. Graft!" Hideki-sensei orders, Haruka passes him the 'harvested' graft, Hideki-sensei get's to work immediately he sows the first end of the graft onto the coronary arteries beyond the point of blockage, he then sows the other end onto the aorta. "Right that should do it restart the heart now!" he instructs loudly, "yes sir" Nana-sensei replies, "Turning off CPB, Warming up the blood to 37 degrees Celsius". The room fell silent, all eyes fell on the heart in front of them, a sound suddenly broke the silence 'BEEP BEEP!', Daisuke-sensei looked at the machine "Heart beat has returned" he reports, a lot of heavy and happy sighs are heard.

"Can you get me the Protamine and a syringe?" Daisuke-sensei askes, Haruka-sensei rushed over the the cupboard with all the bottles in and grabbed the Protamine, "here" she says passing the things to Daisuke-sensei, he measures the amount "injecting" he reports, and syringes it into the lines. Various tubes were then placed into the chest cavity for drainage "Suction" Hideki-sensei orders, after the tubes are removed Hideki-sensei gets to work on wiring up the sternum which took a considerable amount of effort, he then sutured up the chest and with that the operation ended.
I walked out of the observation room and went towards Akira-sensei's operation. As I arrived I saw Koichi-kun coming out of operation, Akira-sensei was leaning against the wall watching the nurses wheel him away. "As expected, the operation was a success hmm?" I questioned him, "naturally" he replied giving me a big grin, "I'm sorry" I apologize, he stared at me blankly for a few minutes "it's fine as long as you understand, also I want a full report of the operation on my desk by Monday he said as he walked of towards his office. "Kana-chan", I swiveled around to see who called me, "ahh Sakurai-sensei good job" I said to him "wanna go grab something to eat?" I giggled remembering I ate his dinner, he nodded to me.

I hurried off towards the female staff locker room to change into my casual wear, I got to the room and opened the door, as usual there were a group of female nurse's gossiping in the corner of the room. I went to my locker and unlocked it, I swung the locker door open and silently changed my clothes. As I was about to leave to meet Sakurai-sensei, I heard some really shocking gossip that the female nurse's were discussing with one another, about Akira-sensei's past, 'ohh well it couldn't be true' I thought as I stormed out of the room.
"Sakurai-sensei" I called out to him "this way", Sakurai-sensei looked at me and smiled, he came over to me and we walked towards the hospital main escalators, I went to step on the first step but my heel slipped over the edge and I was falling, at that moment I was so worried, 'would I be okay', suddenly a reassuring arm curled around my waist and brought me against something warm. It was Sakurai-sensei. As we got to the bottom of the escalators he released me from his grasp, but I was still in shock a little so I swirled a bit. "You okay?" he asked me, "ohh yeah I'm fine sorry about that" I say trying to regain my lost composure, I just made a fool of myself in the hospital lobby, great.

"So which way?" Sakurai-sensei asked me, I pointed to my right "this way hurry up or i'll leave you" I said to him playfully, he laughed at me "got it". We arrived at the restaurant around 15 mins or so later, "table for two?" the waiter asked "please" I say. "Sakurai-sensei can i ask you a question?" I ask, "sure" he reply's nodding. "How well do you know Akira-Sensei?" I ask him, "I know him well, you like him?" he answer's looking up from the menu at me a soft smile on his face, "HIM! NO WAY!" I shout at him, not realizing the atmosphere in the restaurant everyone's eyes were now on me "sorry" I apologize to everyone.
We got in the taxi, "to Kimiko apartment estate please" I say to the driver. I tried to continue the talk with Sakurai-sensei about Akira-sensei "what I wanted to ask you was..." I looked at the driver who was nosing into our conversation, I flashed him a dirty look then leaned over closer towards Sakurai-sensei and whispered "is it true that Akira-sensei's wife passed away due to malpractice in our hospital?", as I moved away I noticed Sakurai-sensei's face darkened, "don't talk about that, I don't know where you heard that but never mention it again!" he said raising his voice at me. That was the first time the gentle Sakurai-sensei actually scared me.

"Little Missy we're here" said the taxi driver gleefully, he sort of reminded me of how I acted at the restaurant not being able to read the situation and all. "Thank you" I said handing him the fair, "keep the change, and umm bye Sakurai-sensei" I muttered, he looked at me and gave a feeble smile. I turned around and retreated into my apartment the words still ringing in my head, 'never mention it again', I sighed and threw myself onto the couch before I knew it I had drifted off.
'Beep! Beep!' hnng I threw my hand to my right, and hit the glass table, I pulled my hand back and winced in pain, drats! that hurt, I forgot that the night before I didn't get into bed 'Beep! Beep!' the alarm still resounding through the rooms. I went to my room and hit the switch on the wall in frustration, the alarm lights disappear, I groaned, now I would have to reset it again.

Feedback:
I am enjoying this :3 Lots of medical terms (specifically equipment) to grapple with but the way that they are introduced in the quick tension filled operation makes it so that you can quickly understand what they do and they also add that element of professionalism, you can feel like you're in the operation which is brilliant ;)
I love this following of the main character after the operation into her personal life, she's a bit of a social klutz and that's been very well written and does make you empathize a lot more with her character which is good ;D (certainly gives me some hope if i want to go into medicine)
And this dark business about malpractice is exciting. I definitely want to see what happens next ;)
Possible scenario: Whei crashes through the ceiling during a romantic moment somewhere. No story is complete without that scenario.
Possible scenario 2: Sakurai-sensei secretly wears bright pink underwear and Kana accidentally finds out...

~ !A short story for Anonymous (obviously Albi!!) Made from your Scenario! ~
Ding Dong! The doorbell chimes, "coming" shouts Sakurai-sensei as he rushes towards the door, he hits his knee on the table as he passed through the kitchen "OUCH!!" he exclaimed to himself, as he got to the door he flung it open to see Kana-chan stood there, "oh come in please" he invited smiling. Kana-chan closed the door behind her as she walked into his apartment, "please wait in here" he says allowing her to enter his room, he left her to attend to his wound from earlier, Kana-chan curiously looks around before sitting on the bed, she puts her hand down besides her but feels something soft, as she inspects it it appears to be pink, "ohh!! what's this!?" she questions pulling up what she has in her hand upwards and stretching it out with both hands, it was a pair of bright pink underwear and at that moment 'BANG' something crashed through the roof!. When Kana-chan could see, after the dust cleared up she saw a large calico Uilus in front of her, she looked around for the pink underwear but she couldn't see it anywhere, as she turned around the uilus had sat down and was wearing :O :O Sakurai-sensei's pink underwear. "What was that bang!!?" shouted Sakurai-sensei entering the room, Kana-chan looked at him and pointed at Whei! Sakurai-sensei could not believe his eyes, he suddenly saw what the uilus was wearing and his face suddenly turned a similar shade of pink :O ohh dear poor Sakurai-sensei. Kana-chan smiled at him and said "so you like pink?" he looked at the floor 'how embarrassing'.
