Eh, so I have to do this survey for school. It's based on a paper I wrote, which is part of this huge project we're doing. Anyway, we're allowed to do it over the internet, so I figured this would be a good option for me. I need 50 people before Monday, so please tell anyone you can about this. It's fine if you ask your family and friends. What I'm asking you guys to do is take a look at this and post your answers. If it's a yes or no question, you can just put "Y" or "N". Same goes for true or false. "T" or "F". As for answers that contain letters, please follow what the question asks and post the appropriate number of letters. Usually this will just be one. Please do not look up any of these questions. These are all opinion and general knowledge-based questions. If you have any questions, please rmail me. Thanks to all who fill this out! 1. Do you know if you suffer from a diagnosed mental disability? (Examples: ADD, OCD, Bipolar Disorder, etc.)
Yes No 2. Would you marry somebody who suffers from chronic depression? Yes No What is chronic depression? 3. Schizophrenia’s affects on the brain are visible.
True False
4. Should health insurance be required to cover treatment of mental illnesses or only cover treatment for physical diseases and disabilities?
Only physical Physical and mental 5. How badly of an effect do you believe ADD has on the average sufferer’s daily life?
1 2 3 4 5 no effect severe effect
6. Would you be close friends with somebody who has Obsessive Compulsive Disorder (OCD)?
Yes No What is OCD?
7. Which famous person had Schizophrenia?
a. Albert Einstein b. Mary Todd Lincoln c. Betsy Ross d. John Lennon
8. Out of the following disabilities, please write down the letter of the ones you are able to identify.
a. Diabetes b. Narcolepsy c. Crohn’s Disease d. ADD/ADHD e. Epilepsy f. Autism
9. Do you believe it would be beneficial to have early childhood screenings for mental disabilities in addition to other routine screenings performed during childhood checkups?
Yes No
10. If you had a child with a disability, which would you want them to remain silent about to avoid the risk of embarra.ssment?
a. Mental disability
b. Physical disability
c. Both
d. Neither

1. Do you know if you suffer from a diagnosed mental disability? (Examples: ADD, OCD, Bipolar Disorder, etc.)
Yes No (I have been diagnosed with ADD.) 2. Would you marry somebody who suffers from chronic depression? Yes No What is chronic depression?
3. Schizophrenia’s effects on the brain are visible.
True False
4. Should health insurance be required to cover treatment of mental illnesses or only cover treatment for physical diseases and disabilities?
Only physical Physical and mental
5. How severe an effect do you believe ADD has on the average patient’s daily life?
1 2 3 4 5 no effect severe effect (My ADD in its untreated state makes me almost completely incapable of fully understanding mathematical computations. Because of this, I nearly failed the 7th and 8th grades. When I'm under the effects of my medication, I'm practically normal but often rather dispirited.)
6. Would you be close friends with somebody who has Obsessive Compulsive Disorder (OCD)?
Yes No What is OCD? (Can I pick the last two results? :D)
7. Which famous person had Schizophrenia?
a. Albert Einstein b. Mary Todd Lincoln c. Betsy Ross d. John Lennon (*guesses*)
8. Out of the following disabilities, please write down the letter of the ones you are able to identify.
a. Diabeetus :D b. Narcolepsy c. Crohn’s Disease d. ADD/ADHD e. Epilepsy f. Autism
9. Do you believe it would be beneficial to have early childhood screenings for mental disabilities in addition to other routine screenings performed during childhood checkups?
Yes No (While some mental illnesses only show up later in life, a lot of them can be detected in early childhood. If I had been screened for ADD earlier in my childhood, my academic life would never have suffered as a result of my condition.)
10. If you had a child with a disability, which would you want them to remain silent about to avoid the risk of embarra.ssment?
a. Mental disability
b. Physical disability
c. Both
d. Neither (Mental and physical disabilities should not be embarra.ssing, as they are conditions that one does not control. It's sort of like being embarra.ssed about your hair color. People don't make fun of someone for being in a wheelchair, so why should someone be teased for having a mental disability?)

1. Do you know if you suffer from a diagnosed mental disability? (Examples: ADD, OCD, Bipolar Disorder, etc.)
Yes No 2. Would you marry somebody who suffers from chronic depression? Yes No What is chronic depression? (But only because I'm never, EVER getting married again =P) 3.
1 2 3 4 5 no effect severe effect There are so many different levels of ADD, its impossible to accurately answer in this way
ADHD e. Epilepsy f. Autism
9. Do you believe it would be beneficial to have early childhood screenings for mental disabilities in addition to other routine screenings performed during childhood checkups?
Yes No
Doctors pimp and push drugs too much as it is as a solution for everything. I wouldn't want to give them the chance to push a pill at a kid who didn't need it just to get a cut of the drug sales. Mental disabilities tend to show up after being triggered anyway, so a generalized screening really wouldn't do much good at all.
10. If you had a child with a disability, which would you want them to remain silent about to avoid the risk of embarra.ssment?
a. Mental disability
b. Physical disability
c. Both
d. Neither More people need to be aware that a mental illness is not something you bring upon yourself, and the shame and embara.ssment that comes attached to it needs to be done away with. There plenty of physical illnesses that are self-caused, yet no one blames the person for those.

It would have saved me years of frustration and misunderstanding if I had been diagnosed with ADD when I was ten instead of nineteen. The meds they put me on immediately made a significant difference. For the first time in my life I was able to sit through a lecture and digest it. Everyone suddenly realized that the reason I was a 'D' student all of my life was because I had ADD, not because I didn't care about my grades. I can't even begin to explore how much I've missed out on in life simply because I was diagnosed too late. 10.
